Purpose of the Form:

This form is for Genetic counselors requesting a copy of a credit or participation certificate from ACMG Education.

Verification Process:

ACMG Education will verify the attendee's participation and the credits being requested.

Important Note for Genetic Counselors:

Before completing this form, please contact the National Society of Genetic Counselors (NSGC) at CEU@nsgc.org to confirm whether your credits have already been reported.

Fees Apply:

Anyone requesting educational credit verification or a reissued certificate will incur an administrative processing fee.

Payment Instructions:

• Submit one payment per course.
• Ensure payment is complete before submitting the request.

Processing Time:

• Requests are processed electronically within two weeks after payment.
• Please allow 3-4 weeks for requests submitted in March due to increased workload during the ACMG Annual Clinical Genetics Meeting.
• Verified and reissued credit certificates will be sent via email.
