Premature ovarian insufficiency (POI) is a condition where ovarian function declines before the age of 40 and affects 3.7% of women. It is a heterogeneous condition with various symptoms and long-term complications, such as osteoporosis, cardiovascular disease, and cancer. The causes of POI are complex, involving genetic, immune, infectious, and iatrogenic factors. While some genetic factors have been identified, more than 50% of cases are idiopathic. This study aimed to investigate the oligogenic basis of POI to improve early diagnosis and treatment.<br /><br />The researchers performed whole exome sequencing and whole genome sequencing to analyze the genetic makeup of 53 patients with POI and 473 controls. They identified eight variant combinations in eight of the patients. Further analysis of two POI-associated genes, ATM and ORC2, showed that co-transfection resulted in greater cell cycle arrest and apoptosis compared to single-transfected groups. Gene burden analysis revealed that patients with POI had a higher frequency of multiple gene variants related to DNA damage repair and meiosis compared to controls.<br /><br />The researchers also used the OVRAL platform to verify the pathogenicity of the RAD52 and MSH6 combination. The study concluded that oligogenic inheritance plays an important role in POI and provides insights into the biological mechanisms behind the condition. However, further research is needed to understand the etiology of POI and identify potential therapeutic targets.<br /><br />This study was supported by the National Key Research and Development Program of China. POI has significant implications for women's health, and understanding its genetic basis can help improve diagnosis and treatment options. Further investigations are necessary to uncover more about this condition and develop effective interventions.
