Health System Direct Contact of Relatives for Cascade Testing: Reach and Initial Acceptability in a Prospective Intervention Study

A study conducted by a health system aimed to determine the effectiveness of a cascade testing program in reaching at-risk relatives for hereditary cancer risk. The program involved a genetic counselor offering to contact relatives directly during the proband's appointment and recommend cascade testing. Consent was obtained from 45% of the 101 relatives approached, and 43% of those consented completed direct contact by phone or letter. The study found that the program reached 1.6 at-risk relatives per proband and was deemed acceptable. The most common reason for probands declining direct contact was the desire to notify their relatives themselves. Concerns about values-related issues with direct contact programs were rare. Among the relatives who did not enroll in the program, the most common reason was incomplete consent. Overall, the results of the study suggest that a health system cascade testing program with direct contact of relatives can effectively reach at-risk individuals for genetic testing.
