This study evaluates the feasibility and initial participation in traceback cascade screening programs for ovarian cancer implemented at three integrated health systems. The programs aim to identify patients with a history of ovarian cancer who did not receive genetic testing at the time of diagnosis and evaluate inherited cancer risk through retrospective outreach. The study finds that traceback programs are feasible and can identify opportunities for genetic testing. The overall uptake of participation in these programs is 23%, with Geisinger having a 13% uptake, Kaiser Permanente (KP) Mid-Atlantic States having a 37% uptake, and KP Washington having a 29% uptake. Each healthcare system designed and piloted its own traceback program, identifying patients through the Virtual Data Warehouse (VDW) Tumor Registry. Inclusion criteria included patients with a history of ovarian, fallopian tube, or peritoneal cancer diagnosed between 1974 and 2021 without electronic health record evidence of comprehensive genetic testing, while exclusion criteria included patients who did not meet NCCN clinical guideline criteria for genetic testing of inherited cancer risk. The study also finds that staff ability to order genetic testing may be associated with higher participation, while barriers to implementation include the need for manual electronic health record review to identify candidates for traceback programs, non-clinical staff calling patients, and the requirement for formal pre-test genetic counseling before arranging test ordering and sample collection. Overall, traceback cascade screening programs have the potential to increase the uptake of genetic testing in patients with ovarian cancer.
