Ensuring equity in the delivery of cancer genetic services: Identifying disparities along the care delivery continuum

This study aimed to identify disparities in access to genetic testing for hereditary cancer syndromes along the care delivery continuum. The results showed that genetic testing uptake differed significantly depending on the workflow, with 11% for referral and 33% for point-of-care (POC) testing. Black non-Hispanic individuals had lower odds of testing compared to non-Hispanic Whites, but this disparity was only present among referral sites and not POC workflow sites. Among referral sites, Black non-Hispanic individuals had decreased odds of genetic counseling.<br /><br />The study drew data from 15 clinical sites across 6 U.S. states, and logistic regression analyses were conducted to evaluate the associations between race/ethnicity and outcomes such as appointment scheduling, genetic counseling, and genetic testing. The study had strengths including a large sample size and examination of multiple outcomes along the care delivery continuum. However, individual-level data on education, income, and insurance were not available, which could have provided further insights.<br /><br />Addressing disparities in genomics and precision medicine is crucial for ensuring equity in healthcare delivery. The study suggests that logistical and systems-level barriers to seeing a genetic counselor may be a primary driver of racial/ethnic disparities in genetic testing for non-Hispanic Blacks. Strategies to reduce these barriers, such as modifying clinical workflows, may help mitigate these disparities.<br /><br />Overall, understanding disparities along the care continuum is essential for addressing inequities in genomics and precision medicine. This study sheds light on the racial/ethnic disparities in the uptake of genetic testing for hereditary cancer syndromes and highlights the need for targeted interventions to improve access and utilization of genetic services among underserved populations.
